Hydroxycobalamin (ohcbl, or B12a) is a natural form of vitamin B12, or one of the vitamin B12 classes. It is a basic member of the cobalamine group. Many of the bacteria that produce vitamins commercially produce vitamin B12 in the form of hydroxycobalamin. Hydroxycobalamin is dark red. Vitamin B12 in the human body is not normally present in the form of hydroxycobalamin. However, in humans, hydroxycobalamin is easily converted into a useful coenzyme form of vitamin B12. The medicinal hydroxycobalamin product is its sterile injection, which is used to treat vitamin deficiency and cyanide poisoning (because it is compatible with cyanide ions).

Hydroxycobalamin is called hydroxycobalamin because the central ion of B12 is connected with hydroxyl group. Although the absorption rate is not comparable to that of cyanocobalan and Mecobalamin, its concentration in the eye is very high, which can greatly relieve visual fatigue and nourish the optic nerve. Therefore, it has a certain curative effect on amblyopia caused by poisoning. In addition, hydroxycobalamin can quickly combine with free cyanide to form nontoxic cyanobalamin in aqueous solution, so its other therapeutic effect is the antidote of cyanide poisoning.
